GLP-1s: My Honest, No-Fluff Take (And What I'd Try Instead)

If I had a dollar for every time someone asked me about Ozempic, Wegovy, or "that weight loss shot everyone's on" this year, I could probably fund my own compounding pharmacy. So let's talk about it — Root Cause Rebel style. No hype, no fear-mongering, just the truth as I see it.

Yes, They Work. That's Not the Question.

Let's start here, because I don't want to be that practitioner who dismisses something just because it's popular. GLP-1 receptor agonists genuinely help people lose weight, and for some — especially those who've felt stuck for years despite doing "everything right" — they can be a legitimate kickstart. I know plenty of smart, root-cause-minded practitioners who use them thoughtfully with their patients. I'm not here to tell you they're evil.

But I am here to tell you they're not the miracle everyone's selling them as. And if something sounds too good to be true, my nurse brain starts asking questions.

Here's What They Don't Tell You

Traditional-dose GLP-1s come with a real list of tradeoffs, and I don't think patients are being given the full picture before they sign up:

  • Nausea, vomiting, and constipation (sometimes severe)

  • Gastroparesis — your stomach literally stops emptying like it should

  • SIBO and other bowel overgrowth issues

  • Significant muscle loss right alongside the fat loss (this one matters more than people realize — muscle is metabolically protective tissue, and losing it isn't a neutral tradeoff)

  • Hair loss

  • Gallbladder disease and, rarely but seriously, pancreatitis

  • Kidney and thyroid concerns

  • Trouble getting in enough nutrients — particularly protein and hydration

And then there's the question nobody wants to sit with: what happens when you stop? For most people, the weight comes back — often quickly — unless the underlying root causes were addressed the whole time you were on the drug. So are you supposed to stay on it forever? Because that's a fantastic business model for pharmaceutical companies and a pretty big unknown for your long-term health. We simply don't have the decades of safety data to know what indefinite use does to a body.

The Part That Really Gets Me

I had a client reach out recently — thin, not remotely looking for weight loss — because she'd heard people rave about how much their brain fog and inflammation improved on a GLP-1. And listen: I have so many better ways to get you there. Reducing systemic inflammation and clearing brain fog isn't a GLP-1 party trick — it's what happens when you actually drain, detox, and support the body's own clearance systems. That's root-cause work. You don't need a weekly injection and its side effect list to get your brain back online.

Why the Sequence Matters More Than the Drug

Here's the piece I think gets lost in every GLP-1 conversation: these medications are a signal. They're telling your hypothalamus and gut "stop being hungry." That's it. Add in supporting digestion, gut motility, and blood sugar regulation, and you're working with your physiology instead of overriding it with a pharmaceutical.
